Freshly prepared lunch and snacks, with allergies, textures, and dietary needs handled with real care.
Personal care & 1:1
Respectful assistance with daily routines and personal care, scaled to the support each guest needs.
Community outings
Regular, well-supervised trips into the community, parks, the library, local events, so the day isn't lived indoors.
Social connection
The thing families value most: friendship, routine, and a genuine sense of belonging.

Welcome & morning check-in
A friendly greeting and a morning health check from our nurse, then coffee and conversation.
9:30 AM
Morning activities
Art, music, movement, or a life-skills session, chosen with each guest's interests in mind.
12:00 PM
Lunch together
A fresh, nutritious meal shared around the table, the social heart of the day.
1:30 PM
Outings & afternoon fun
A community outing or an afternoon of games and friendship, with rest whenever it's needed.

Wind-down & send-off
A calm close, a note home about the day, and a safe hand-off to family or transport.
Is My Choice the right fit?
We support adults who:
- Are 18 to 59 years old
- Have autism or another intellectual or developmental disability (Down syndrome, cerebral palsy, and others)
- Benefit from daytime supervision, structure, and engagement
- Live in Luzerne County or the surrounding NE Pennsylvania area

How it's funded
For most families, cost is not the barrier they expect.
My Choice is built to work with Pennsylvania's Medicaid waivers and county-funded programs for adult day services.
Adult Autism Waiver
Pennsylvania's waiver specifically supporting adults with autism, often covering adult day services.
Consolidated, Person/Family Directed Support (P/FDS) & Community Living waivers
The Office of Developmental Programs (ODP) waivers that fund day programs for adults with intellectual and developmental disabilities.
County & ODP-funded programs
We'll help you understand county and state options, and connect with your Supports Coordinator.
We'll review your family member's funding with you, step by step. If something isn't covered, we'll tell you plainly.
